Abstract
As youth with psychiatric conditions move from adolescence to adulthood, not only do they face developmental challenges, but they also have to negotiate changes to the way they receive care. They must gain understanding of their diagnosis, find new providers, and learn how to independently manage their own health. Pediatric and adult psychiatrists are often left feeling wholly unprepared to navigate these changes with their patients. This chapter details how to educate and train the next generation of physicians so that they might successfully and effectively help youth traverse this period of transition.
