To the Editor: Dr. Robert Neer, in his succinct and rather complete summary on the effects of thiazide diuretics on calcium metabolism (N Engl J Med 286:767, 1972), implied that the occasional occurrence of hypercalcemia resulting from thiazides does not require the presence of the parathyroid glands. We agree, having seen hypercalcemia (level of 12.6 mg per 100 ml) during treatment with chlorothiazide in a patient with hypoparathyroidism. It should be pointed out, however, that the reduction in urinary calcium caused by thiazides is not independent of parathyroid function, and occurs to a much lesser degree in patients with hypoparathyroidism,.
