Abstract
The Hoffman-Vidal external fixation apparatus was employed in the treatment of 23 complex tibial fractures from 1978 to 1982. Average time in the apparatus was 7.1 weeks. Healing time was prolonged with increased severity of the injury. Complications consisted of pin tract infections, osteomyelitis, skin sloughs and delayed union. It is felt that the high complication rate in the series is related to the severity of the injury and the technical application of the Hoffman apparatus. The Hoffman apparatus remains an important tool in the treatment and the limb salvage of severe tibial fractures. The apparatus maintains anatomic alignment of the fracture while allowing accessibility for wound care.
