Abstract
Care should be taken to avoid injury to the urinary tract during gynecological operations to prevent fistula formation. Preoperative risk factors should be identified. Inadvertent urinary tract injuries should be recognized and repaired under the same anesthesia, since the best chance of preventing fistula formation is at that time. For repair of an intraoperative injury, the authors recommend to evaluate the extent of the injury, to adequately expose and access the injury, to carefully close with an interposition flap, and to provide uninterrupted prolonged postoperative drainage. Intraoperative ureteric injury is better repaired by direct approximation, but other procedures are required in severe injuries.
