Abstract

The primary purpose of this discussion is to emphasize the clinical importance of differentiating genetic diabetes mellitus from the primary hyperlipidemic states, which can frequently cause a secondary form of hyperglycemia. As illustrated here the electron microscopic diagnosis of diabetic microangiopathy, by providing an independent marker of diabetes mellitus, has proven to be of great assistance in this differential diagnosis. The distinction between these two disorders with their identical presenting chemical abnormalities can lead both to more accurate diagnosis and to a more rational and effective therapeutic approach to the treatment both of hypertriglyceridemia and of hyperglycemia.
