The role of F-18 fluorine fluorodeoxyglucose (F-18 FDG) positron emission tomography/computed tomography (PET/CT) in the functional imaging of malignancies is well-known. Intraventricular germinoma is a rare central nervous system tumor. We describe a 25-year-old man with symptoms of diabetes insipidus, orthostatism, and visual changes evaluated by F-18 FDG PET/CT and MRI. There was an intense FDG hypermetabolic mass in the fourth ventricle, which was surgically removed. The pathology confirmed a germinoma. FDG PET/CT was useful in excluding distant metastases in the remainder of the neural axis.
