Abstract

The publisher regrets an error was made in the authorship list and would like to correct this error. The correct authorship is listed above. The publisher also regrets that the Conclusion section was not included within the Abstract. This should read: “Conclusion: Identification and understanding of the relationships between factors that contribute to handoff success and failure is paramount to improving healthcare communication. This theoretical model provides a foundation for further research, potential interventions, measurement, and organizational policy.” The publisher would like to apologise for any inconvenience caused.
