Epidural catheter infusion for post-operative analgesia for major spine surgery

Abstract

Adequate post-operative analgesia following major spine surgery is crucial for favorable outcomes. Multimodal analgesia is the preferred analgesic approach for post-operative pain management following spine surgery. In major and complex spine surgery and for certain surgical patient populations continuous perioperative epidural analgesia can be a powerful addition to improve pain control. As a component of an Enhanced Recovery Protocol it can attenuate the surgical stress response, help facilitate early mobilization, and assist in early functional recovery. This chapter will address the rationale for utilization of an epidural catheter infusion for post-operative analgesia, discuss the benefits, risks, limitations, patient selection, medication choice, necessary equipment, and methods to integrate this modality into an Enhanced Recovery Protocol.
