Abstract

Concussion or mild traumatic brain injury is a common injury among all ages and particularly concerning in youth sports and other physical activities. While the clinical definition of concussion continues to be refined, our knowledge of the pathophysiology is rapidly expanding. Ion fluxes, interference with cellular energy production, and resultant changes in gene expression and axonal function all contribute to signs and symptoms. In this chapter, we discuss the concussion clock: the diagnostic and management protocols that take place within minutes, hours to days, and weeks to months. Aspects of physical examination, laboratory testing, and neuroimaging guidelines are reviewed. The initial approach to treatment and acute medical syndromes are discussed with evidence-based recommendations.
