Musculoskeletal pain may be the initial presentation of various malignancies. Leukemia presenting with bone pain is not uncommon. This case illustrates that periosteal reaction can be one of the initial radiological signs of leukemia and an immunohistochemical study with karyotyping is necessary to confirm the diagnosis.
